Also be sure to state what your contribution was to a project and describe your role - don't just include a couple of screenshots or a link....

A killer portfolio! A good portfolio will include more than just images and links. It includes details explaining what you did and how you did it. Also helps tremendously to include supporting documentation such as wire frames, design specs, etc.
